This Material Request Form is used to request materials that you would like the Cozby Library and Community Commons to purchase and are not available in our current library catalog. Check the online catalog for availability before filling out this form. Requests for items not yet published or released will not be accepted more than 30 days prior to the publication or release date. All requests will be seriously considered within the context of the collection development policy and within materials budget constraints.
